MEDICAL & PRESCRIPTION DISCLAIMER
Medical Evaluation and Prescribing
Prescription services coordinated through Daily Dose Wellness Club require an evaluation by a licensed healthcare provider. Your provider will review your medical history, current medications, allergies, symptoms, and other relevant clinical information to determine whether additional evaluation or treatment is appropriate.
A consultation does not guarantee a prescription, treatment, or access to a particular medication. All prescribing decisions are made independently by the treating healthcare provider, within their professional scope and in accordance with applicable federal and Texas law.
Compounded Medications
Compounded medications may be considered for an individual patient’s medical needs when permitted under applicable law.
Compounded medications are not FDA-approved. The U.S. Food and Drug Administration does not review them for safety, effectiveness, or quality before marketing. A compounded medication is not an FDA-approved product.
If treatment is recommended, your provider will discuss its purpose, potential risks, alternatives, and monitoring requirements. Individual results vary, and no treatment outcome is guaranteed.
Medical Supervision and Follow-Up
Patients should follow their provider’s instructions and promptly report unexpected side effects, adverse reactions, or changes in their medical condition. Additional consultations, laboratory testing, or an in-person examination may be required before treatment begins or continues.
Do not start, change, or discontinue a prescribed medication without consulting your treating provider.
Eligibility and Texas Service Area
Eligibility is determined through an individualized medical evaluation and applicable legal requirements.
Patients must provide accurate registration and medical information, complete required intake forms, and participate in the necessary clinical evaluation. Additional age and consent requirements may apply to the particular service.
You must be physically located in Texas during your telehealth consultation. Daily Dose currently limits prescription services to eligible Texas residents. If medication is prescribed for shipment, a valid Texas shipping address is required.
Website Requests and the Provider-Patient Relationship
Browsing this website or submitting an appointment request does not, by itself, establish a provider-patient relationship.
A provider-patient relationship is established through the treating provider’s clinical process in accordance with applicable law. Receiving an evaluation does not guarantee that treatment or medication will be recommended.
Telehealth Services
Some services may be delivered through telehealth. Telehealth has limitations, including potential technology disruptions and the inability to perform certain physical examinations.
Your provider may determine that an in-person examination, laboratory testing, imaging, or referral is needed. You are responsible for having a compatible device, reliable internet connection, and a suitable private location for your appointment.
Any required telehealth consent will be obtained through the clinical intake process. This website disclaimer does not replace informed consent for telehealth or treatment.
Telehealth and website request forms are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Identity Verification and Medical Intake
Daily Dose requires identity verification before telehealth services. You may be asked to provide valid government-issued photo identification and your current residential address through the designated intake process.
Provide only identification that belongs to you and ensure that your registration information is accurate and current. Submit identification, medical records, and other sensitive information through the designated intake process rather than general website forms or social media messages.
Information collected during registration and care is handled in accordance with applicable privacy requirements and the relevant privacy notices.
Privacy and Your Health Information
Our HIPAA Notice of Privacy Practices explains how protected health information may be used and disclosed, your rights regarding that information, and how to submit privacy requests or complaints.
As described in that Notice and permitted by applicable law, health information may be used or disclosed for treatment, payment, healthcare operations, and other authorized or legally permitted purposes. Uses or disclosures requiring your written authorization will be handled through a separate authorization process.
Reading this disclaimer, using the website, or acknowledging receipt of the HIPAA Notice does not replace an authorization required by law or waive your privacy rights.
Please review our HIPAA Notice of Privacy Practices for the full explanation of our health information practices. Our website Privacy Policy provides additional information about information collected through the website.

Fees and Recurring Programs
Applicable consultation fees, treatment charges, and cancellation terms will be provided before purchase or enrollment.
If you enroll in a recurring program, review its specific billing, renewal, and cancellation terms. Enrollment or payment does not guarantee that medication will be prescribed or renewed. Continued treatment depends on your provider’s clinical judgment and any required follow-up evaluation.
